I. Telehealth Consent

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a licensed healthcare provider and a patient who are not in the same physical location. Well RX uses secure, encrypted systems to protect the privacy and integrity of your health information during telehealth interactions.

Expected Benefits of Telehealth:

• Improved access to weight loss management and medical care regardless of your location
• More efficient medical evaluation and management
• Access to specialist expertise that may not be locally available
• Continuity of care with your established Well RX provider

Possible Risks of Telehealth:

• Insufficient image resolution or connectivity issues may affect clinical decision-making
• Technical equipment failures could cause delays in treatment
• In rare cases, security protocols could fail, resulting in a breach of privacy
• Incomplete transmission of medical records could result in adverse interactions or decisions

By using our services, you acknowledge that telehealth consultations carry the same privacy protections as traditional in-person medical visits, you retain the right to withdraw your consent to telehealth at any time, and you have the right to inspect your medical records and request copies.

II. HIPAA Consent

Your health information is confidential and will only be shared as necessary to provide your care. Well RX and its affiliated providers share information with other healthcare providers, laboratories, and pharmacies involved in your treatment. Business vendors who support our operations may access protected health information solely to perform their services and are bound by HIPAA confidentiality obligations.

Your confidential information will not be used for marketing or advertising of products, goods, or services without your explicit written authorization.

III. Financial Consent

By enrolling in Well RX, you authorize Well RX to maintain your payment information on file and to charge that payment method for services rendered, including recurring subscription fees. Auto-renewing programs will continue to bill until you explicitly request cancellation in accordance with our Refund Policy. You authorize Well RX to submit claims to insurance payers and to release medical records as necessary to process your orders and obtain payment.

There are no refunds or exchanges on prescription products once dispensed. Please review our Refund Policy for full details.

IV. Shipping Authorization

Medications are dispensed in accordance with state and federal law and with pharmacist approval. Your medication is considered dispensed and your order is considered complete when it is signed out for shipping — not when it arrives at your address. By accepting these terms, you assume responsibility for any shipping delays or errors that occur after your order has been signed out by the pharmacy. Well RX will make reasonable efforts to assist in resolving shipping issues with our partner pharmacies.

V. Modifications

Well RX reserves the right to modify these consent terms as needed for legal or business reasons. Continued use of the platform constitutes acceptance of any revised terms.
